A multidisciplinary, multifaceted approach to disease management that incorporates the health system, the provider, and the patient is supported in the literature. A simulation model using a US health care payer perspective was designed to predict changes in lipid levels (LDL-C, TG, high-density lipoprotein cholesterol, and total cholesterol) and long-term CHD risk. Data were collected from specialists online and from PCP attendees at live symposia across the United States. To effectively lower this significantly increased burden of disease, a comprehensive multidisciplinary approach to chronic disease management is required. The study has several strengths and limitations. Improvements were seen for more than 6 months of initial practice change, including compliance with annual glycosylated hemoglobin (HbA1c), lipid, blood pressure, and foot checks. A 20-year model of direct costs and quality-adjusted life-years (QALYs) was created.
