Diabetes is the fastest-growing chronic disease in the U.S., but experts says there is a relatively simple and inexpensive way to reduce the soaring cost of treating it: Get patients to take better care of their feet. Costs were estimated using detailed data on resource use and Medicare payment methodologies. Data were collected by reviewing the medical record of each patient for the whole year. The annual number of visits of these diabetic patients was 6.2% of the total outpatient visits of all patients due to all causes during the one-year study period. In contrast, an economic model predicts that intensive therapy, as compared with conventional therapy, could reduce blindness from 34 to 20% or by 41%, end-stage renal disease from 24 to 7% or by 71%, and lower-extremity amputations from 7 to 4% or by 43%. IAPP has been postulated to have a role as a modulating factor in glucose homoeostasis and to be involved in the pathophysiology of diabetes. There are approximately 2000 beta-cells per islet, and approximately one million islets of Langerhans scattered throughout the pancreas. Frequently they form fibers or plaques whose presence is correlated with the pathology for many diseases, including Alzheimer’s, Parkinson’s, and type II diabetes. Total insulin secretion per 180 min was elevated by 30% in GDM patients (35 +/- 3 pmol/l) versus control subjects (27 +/- 1 pmol/l) (P < 0.05), but increased even more (190-250%) in obese insulin-resistant women, compared with all other groups (68 +/- 7 pmol/l, P < 0.0005).
